There are several ways that patients can access information about their health:

Personal health records (PHRs): Some patients may choose to create a personal health record (PHR), which is a record of their health information that they can manage themselves. PHRs can be useful for tracking health information, such as medications and allergies, and for sharing this information with healthcare providers.
Requesting copies of medical records: Patients can also request copies of their medical records from their healthcare provider. In many cases, patients will need to sign a consent form and pay a fee to obtain copies of their records.
