Long-term post-acute care (LTPAC) is a type of care that focuses on the recovery, rehabilitation, and management of chronic symptoms. This type of care is generally provided to elderly patients or young patients with disabilities. The LTPAC services include a wide range of services from institutional services (care provided in hospitals & nursing homes) to home and community-based services. Long-term care services are provided to patients who need a longer hospital stay. In contrast, post-acute care services are provided to patients post-hospitalization as continued medical care to ensure a full recovery.
