The Xealth Digital Care SMART on FHIR app is a digital health solution that enables physicians and care teams to order, deliver, and monitor digital health tools from within the electronic health record (EHR) system. The app is available for download from the EHR’s app store and can be configured by the health system team to integrate directly into the provider’s workflow. The integration allows Xealth to leverage data in the EHR to suggest and enroll patients into relevant programs.
The clinical rules engine in Xealth can automatically distribute tools and assets to specified patient groups or present relevant solutions within the patient record for manual ordering. The app can also be triggered by EHR alerts during a patient visit, making it easier for clinicians to order relevant digital health options.
Once ordered, Xealth manages all aspects of delivery and enrollment, either by sending assets directly via email or SMS, or by placing them in a digital care section of the patient portal. The app also tracks patient engagement and can send reminders and nudges if engagement is low.
Xealth brings monitoring information back into the health system, allowing care teams to follow the patient’s progress and drive interventions as needed. The app even embeds third-party dashboards within the EHR, making it easy for clinicians to view patient information without leaving the patient record. Data such as PHQ9 scores or blood pressure readings can be stored in the patient’s chart for easy consumption by the clinician, allowing for downstream workflows like notifications, best practice alerts, and patient flags to be generated within the record.
