Your Client Care Coordinator will develop and manage a tailored plan of care in partnership with you and your hospital discharge planning staff to start you off on your road to recovery.

Your nurse-led team of caregivers can assist you with medication support, meal preparation, housekeeping, personal care, and transport services to your follow-up appointments. Home nursing services are also available for your more complex care needs. With no lock-in contracts or commitments, My Care Solution offers flexible hospital to home care options that allow you to increase or decrease your support as you see fit.
