Fonemed offers the first disease management program based on clinical protocols and best practice guidelines specifically designed for remote use in managing chronic diseases such as:

• Asthma
• CHF and
• Behavioral health

Our disease management program, developed with support from Canada’s government, draws on the US Coleman, Wagner, and Veterans Administration models, as well as the Whole Health Program of Great Britain and more than 17 years of our own experience providing remote care in the telehealth industry.

Using telehealth as a key element in the continuum of care, we improve the patient experience, stratify patient populations, provide expanded touchpoints, and significantly reduce costs.

We support newly diagnosed patients as well as patients at every stage of their diseases. This includes both short-term programs to reduce hospital readmissions and longer-term disease management.
