In general, Hospitals provide acute care services, then often discharges to a Rehab Facility that offers transitional care for patients who no longer require to be in a hospital but who aren’t quite ready to function on their own. Once a patient improves enough to go home, the physician can give orders for the patient to continue to receive therapy at home from ADMT Solutions Home Health. Our team delivers excellent care that allows patients to experience healing and independence.

Physical Therapy

Physical therapy for the preservation, enhancement, or restoration of movement and physical function impaired or threatened by disease, injury, or disability that utilizes therapeutic exercise, physical modalities assistive devices, and patient education and training.

Occupational Therapy

Occupational therapy for the engagement in meaningful activities of daily life (such as self-care skills, education, work, or social interaction) to enable or encourage participation in such activities despite impairments or limitations in physical or mental functioning.

Speech Therapy
