Evara Health faced funding challenges in implementing remote patient monitoring (RPM) services due to a patient demographic predominantly on Medicaid or uninsured. In 2021, they secured funding from an FCC telehealth grant to initiate an RPM program in collaboration with Health Recovery Solutions.
The organization initially integrated RPM with its Medical Home@Home (MH@H) service, leveraging teams of medical assistants to visit homebound patients with chronic diseases or recent hospital discharges. The program expanded to include clinic-referred patients and a new cohort for pregnant and postpartum individuals.
Results showed improvements of 5-20% in high blood pressure and A1c rates across the enrolled patient population. The RPM technology also played a critical role in identifying life-threatening situations, such as a patient experiencing a heart attack and an expectant mother with elevated blood pressure indicating early stages of eclampsia.
To succeed with RPM implementation, Evara Health emphasized the importance of defining the problem, having a well-documented rollout plan, and securing a clinical champion to encourage staff participation. Additionally, addressing funding pathways for ongoing service costs and potential reimbursement was highlighted as crucial for sustained success.
