Approved care plans – these plans are reviewed on regular basis and suggestions will be given to improve the existing plan by the provider. Early identification – the team is able to identify medication use, life-threatening vitals and more. Monitoring – patients’ progress is tracked so that providers’ can keep up with the treatment plans and goals.
