Providence Launches Remote Patient Monitoring Program for Chronic Care
The collaborative initiative between Cadence and Providence introduces remote patient monitoring (RPM) and responsive virtual care capabilities to clinics spanning seven states. Providence, a nonprofit Catholic health system encompassing 52 hospitals and over 900 clinics aims to bolster chronic disease management through this partnership. Cadence, a health technology company, contributes RPM solutions and a responsive virtual care program, enabling remote vital sign monitoring and patient care. This novel approach combines technology with a clinical care team led by nurse practitioners to assess patient vital signs outside traditional clinical settings. Pilot implementation of the RPM program in two Washington clinics, St. Mary Cardiology and Lacey Family Medicine, in June 2022 has yielded promising results. Of the 300 participating patients, 89 percent consistently monitor their vitals, with only 0.2 percent requiring engagement with Providence clinicians. Moreover, the program has increased adherence to guideline-directed medical therapies (GDMT) and reduced blood pressure levels in hypertensive patients, underscoring its potential benefits.
