These guidelines would help the clinicians to
- Identify what CCM codes & modifiers are available for use by your healthcare setting
- List possible billing errors and how to avoid denials on CCM claims
- Determine an effective/efficient process for communication between care team & billing
- Develop process to identify who will cover CCM services other than Medicare
- Outline what options are available when a patient cannot/refuses to pay co‐pays
- Identify other audiences to share and this information and collaborate with in this area
- List actions items to take in response to this training
