Addressing Social Determinants of Health (SDOH) in Healthcare Part 2
Numerous research studies consistently underscore the substantial contribution of social determinants of health (SDOH), ranging from 30% to 55%, to health outcomes. Recognizing the profound impact of SDOH on physical and mental well-being, various healthcare entities, including hospitals, health plans, and federal and state programs, are increasingly acknowledging the significance of capturing SDOH data to provide comprehensive patient care. This update introduces a new element of performance (EP) dedicated to screening for social determinants of health. Healthcare organizations are now mandated to evaluate patients' SDOH and formulate plans to address identified needs. They introduced the Community Health Access and Rural Transformation (CHART) Model in January 2021, designed to enhance healthcare access and address SDOH, particularly in rural areas. CMS aims to support community-based initiatives, including improved transportation, housing, and access to healthy food. Through the integration of SDOH into accreditation standards and value-based care programs both The Joint Commission and CMS underscore the critical role of addressing SDOH in healthcare.
