Creative RPM Programs Tackle Chronic Care Management

In Atlanta, a pioneering partnership between Rimidi, a digital health company, and the Brighter Day Health Foundation is launching remote patient monitoring programs in local churches to address chronic care management among underserved populations. This initiative, initially implemented in churches like Impact Church and World of Faith Family Worship, provides weekend clinics equipped with RPM technology to assist church-goers in monitoring blood pressure and managing their health. With a focus on areas predominantly African-American and an average age of 55, about 350 participants engage in this program, receiving connected devices for home monitoring and access to healthcare professionals for personalized care plans. This innovative approach aims to bridge healthcare gaps by meeting communities where they are, fostering healthier living and managing chronic conditions effectively.
