How a Total EHR System Overhaul Can Support Value-Based Care
Since patients typically have more than one issue, Glaser said there could be algorithms to form a “master plan.” The master plans would touch on resolving comorbidities and put the entire care team on one plan, connecting all of the specialists.
As a result of a patient seeing a handful of specialists, the plan must be interoperable between health systems and travel with the patient. The new EHR must also be adaptable to change and remind users of upcoming and overdue activities. Additionally, it must suggest potential changes when the patient’s condition changes and keep all specialists updated.
While the EHR is able to document a visit, retrieve lab results, and send a prescription out to a pharmacy, Glaser said it needs to be more than just a document. It should be a system that has intelligent capabilities and can properly execute many jobs at once.
